10 percent fewer obese after 6-month study

Could a clinic-based intervention that assists in dealing with childhood obesity be scaled down into an easily-taught, community-based program? Yes, according to a new feasibility study conducted by researchers at Temple University's Center for Obesity Research and Education and published in the journal, Pediatrics.

Working in partnership with UnitedHealth Group and the Greater Providence YMCA, the Temple researchers conducted a six-month program on weight loss and management for 155 children and their parents or guardians in Providence, R.I.

"We've known for decades that there are effective treatments for childhood obesity, but they are labor-intensive, expensive and not very accessible," said CORE Director Gary Foster, who developed the study and designed its curriculum along with a scientific advisory board. "We were interested in seeing if a clinic-based program could be diluted and implemented in the community."

Ninety-two percent of the children who participated in the program through the YMCA were obese and almost half were at or above the 99th percentile.

"The 99th percentile basically means you're off the charts," said Foster. "So with almost 50 percent who participated in this study in that category, it indicates to me that there is a lot of pent up demand for these programs in the community."

During the study, the children and their parents/guardians met in 12 face-to-face group sessions at the YMCA and in 12 home sessions in which the parents/guardians conducted a lesson based on a provided handout.

During the sessions, the children were advised on methods to limit or avoid foods such as cookies, candy, sugar sweetened beverages and fried foods.

"Each week, families were given specific goals around eating, activity and tracking progress," said Foster.

Foster said the following week, the group would problem solve about the task: how it went, what was easy and/or tough in accomplishing it. The group also discussed increasing physical activity, getting social support for their goals and dealing with peer pressure, such as friends who are drinking soda or eating pizza.

At the end of the six-month study, the researchers found that 8 percent fewer children were at or above the 99th percentile, while 10 percent fewer were in the obese category.

"The fact that 10 percent fewer children were obese after the program is impressive," said Foster. "If a program like this were implemented across the country, there would be 10 percent fewer obese children. That's a huge impact with significant health and economic implications."

Foster said the fact that none of the group facilitators involved in the study had ever treated childhood obesity before demonstrates how easily the program can be brought into the community through schools, churches and other community organizations.

"Does it work as well as if we had treated the children intensively in the clinic? No," said Foster. "It's about a half to two-thirds as effective. But it is more cost-efficient and more accessible to the millions of families who need help, and if you can get 10 percent of the children to not be obese as a result of this program, that's a big success."

What is the key to an effective ERM program?

Culture.

That?s according to Diana Del Bel Beluz of Risk Wise, Inc. Her session at the 2012 RIMS Canada Conference focused on the culture of an organization and how it can make or break a company?s ERM program.

?What is enterprise risk?? she asked. ?Events or circumstances that could influence either the organization?s ability to achieve its mission or strategic objectives or its reputation, strength and viability.?

The purpose of risk management:

  • Establish strategic context
  • Monitor and review risks
  • Respond to risks
  • Identify risks
  • Assess and prioritize risks

?We do these things to communicate and align to risk appetite,? said Del Bel Belluz.

She stressed that corporate culture is the actions of leaders ? observable artifacts, shared values and tacit assumptions. It is this culture, she says, that is the number one ingredient for effective risk management

But what is risk management? ?The culture, processes and structures that are directed towards realizing potential opportunities whilst managing adverse effects,? said Del Bel Belluz.

To her, the traits of a healthy risk culture are:

  • Accountability ? ?It?s not about blaming, it?s about understanding why it went right or why it went wrong.?
  • Open and inquiring
  • Performance oriented
  • Prepared and ready
  • Collaborative
  • Vigilant
  • Innovative
  • Adaptable and resilient

?To implement sustainable risk management you need to create a plan, have a strategy, create buy-in
and have implementation ? leadership, communication, for example, educating, listening, coaching,? she stressed.

There are three sources of resistance, however. They are:

  • Rational 20% ? the business case (costs, benefits)
  • Emotional 60% ? self-interest, fear, comfort, attachment
  • Political 20% ? shift in power structure

Del Bel Belluz then breaks down the categories of resistance:

Rational resistance

  • Establish urgency
  • Form guiding coalition
  • Create vision
  • Communicate vision
  • Empower employees to act
  • Generate short term wins
  • Consolidate gains and produce more change
  • Anchor new approaches

Emotional resistance

?You have to revert down to the bottom of the hierarchy when dealing with emotional resistance,? said Del Bel Belluz.

Political resistance

To best explain political resistance, Del Bel Belluz referred to a famous quote from Niccol? Machiavelli, which states:

There is nothing more difficult to execute, nor more dubious of success, nor more dangerous to administer than to introduce a new system of things: for he who introduces it has all those who profit from the old system as his enemies, and he has only lukewarm allies in all those who might profit from the new system.

Whether you fully agree with Del Bel Belluz or not, one thing is certain ? culture most definitely plays a part in establishing an effective enterprise risk management program. How much of part depends on the company. But those who feel culture is insignificant to ERM may find, sooner or later, that their program is deeply flawed.

The U.S. has deployed an FBI investigation team and drones to Libya to search for those responsible for the murder of the U.S. ambassador and three other Americans. NBC's Ayman Mohyeldin reports.

Updated at 4:15 p.m. ET:KHARTOUM -- Sudan has rejected a U.S. request to send a platoon of Marines to bolster security at the U.S. embassy outside Khartoum, the state news agency SUNA said on Saturday.

The U.S. ordered all family members and non-emergency personnel out of Khartoum as well as Tunis, Tunisia, posts, State Department spokesperson Victoria Nuland said Saturday afternoon. The State Department also issued travel warnings to U.S. citizens in both countries.

Earlier Saturday, a U.S. official,?who spoke on condition of anonymity because the official was not authorized to disclose details on the troop movement,?said?Sudan's?objection held up the?deployment of 50 Marines. A U.S. official said?the Marines had already set off for Khartoum but had been called back pending further discussions with Sudan.

Nuland earlier Saturday didn't speak about the Marines but acknowledged?Sudan had "recommitted itself both publicly and privately to continue to protect our Mission, as it is obligated to do under the Vienna Convention."

"We are continuing to monitor the situation closely to ensure we have what we need to protect our people and facility," Nuland said.

On Friday, around 5,000 people protested against a film that insults the Prophet Muhammad, storming the German embassy before breaking into the U.S. mission.

They also attacked the British embassy. At least two people were killed in clashes with police, according to state media.

In Tunis, four?people were killed and 46 were wounded, the Tunisian government said, after police gunfire near the U.S. Embassy in the North African city that was the model for last year's pro-democracy revolutions.

Police fought hundreds of protesters who smashed windows, hurled petrol bombs and stones at police from inside, and started fires in the embassy and to a gym and a neighboring American school. A Reuters reporter saw police open fire on protesters forcing their way into the embassy building.

A U.S. official told Reuters on Friday that Washington would send Marines to Sudan to improve security at the embassy located outside Khartoum.

"Sudan is able to protect the diplomatic missions in Khartoum and the state is committed to protecting its guests in the diplomatic corps," Sudanese Foreign Minister Ali Ahmed Karti told SUNA.

The violent protests in response to an anti-Islamic film have been spreading across the Middle East and the North Africa region, with attention focused on U.S. embassies and offices. NBC News' Jim Maceda reports.

Sudan beefed up security at some missions on Saturday. A riot police truck was parked in front of the deserted German embassy, which protesters had set on fire on Friday. An Islamic flag raised by the crowd was still flying. Three officers manned the main gate.

More than 20 police officers were sitting in front of the U.S. Embassy.

The film, which depicts Muhammad as a womanizer and charlatan, was made in the United States, and Muslim outrage has led to crowds assaulting U.S. diplomatic missions in a number of Arab countries.

U.S. authorities are interviewing a California man suspected of making an anti-Islamic film that has sparked violent protests across the Middle East.

Sudan has also criticized Germany for allowing a protest last month by right-wing activists carrying caricatures of Muhammad, and for Chancellor Angela Merkel's award in 2010 to a Danish cartoonist who had depicted the prophet, triggering unrest across the Islamic world.

President Omar Hassan al-Bashir has been under pressure from Islamists who feel the government has given up the religious values of his 1989 Islamist coup.

The Sudanese government had called for protests against the film, but peaceful ones. President Barack Obama's administration said it had nothing to do with the movie, which is little more than an amateurish video clip and appears to have been made in California.

Find My iPhone Leads To Arrest Of Suspects In Atlanta Armed Robbery Case

After five women were held up at gunpoint near the Biltmore Hotel last night in the Georgia state capital, police tracked one of their stolen iPhones to an apartment and nabbed the suspects. A tipster tells us that police used Find My iPhone, the service that Apple makes available to iCloud users who also own iPhone, iPod touch, Mac, and iPad hardware,?to track the device. Find My iPhone has helped in arrests a number of times in the past, including in an armed robbery case reported by the L.A. Times last November.
